Reminders, Announcements, and Information
High School
12th Grade Meningitis Vaccine:
All students in 12th grade must have a 2nd Meningitis vaccine. Students risk exclusion from school if the vaccine date is not provided before the 1st day of 12th grade.
11th Grade Physicals:
Students in 11th grade must submit a physical. Students may be denied entry into 12th grade if the 11th grade physical is not completed. Sports physicals in 11th grade meet the requirements for the 11th grade physical.
Middle School
PA State Mandates
6th Grade students must have a physical and scoliosis exam during the 6th grade year. Please get an updated copy of your child's vaccine record from your child's primary care provider. A booster dose of tetanus, diphtheria, and pertussis (Tdap) and one dose of the meningococcal vaccine are required for 7th grade. Students who have not shown proof of Tdap and the meningococcal vaccine risk exclusion from school per the Pennsylvania Department of Health Guidelines. Please send your child's completed paperwork to Mrs. Sheerer by June 1, 2021. Physical forms can be found under the home tab on the main page of the health services site.
7th Grade students must have a dental and a scoliosis exam during the 7th grade year. Please send your child's completed paperwork to Mrs. Sheerer by June 1st . Dental forms can be found under the Department - Health Services = Resources - Forms on the NPSD website (www.npsd.org).
Intermediate School
We would like your child to have a safe and healthy school year. To ensure this, please take a moment and review the following procedures from North Pocono Intermediate's Health Office.

Any medication that your child needs to take while at school (prescribed or over the counter) requires completion of the Authorization for Medications During School Hours form by a Physician/Practitioner. The completed form and the medication need to be brought to the school by a designated adult. Please do not send any medication to school with your child.

Students may bring cough drops accompanied by a parent note that is effective for one week. The school nurse does not provide cough drops.

Keeping a change of clothes in your child’s locker is encouraged. Accidents happen (spills, rips from playground, etc.) and we want your child to be as comfortable as possible throughout their day.

If your child needs to be excused from physical education due to a medical reason they will also need to sit out of recess.

Please be mindful of keeping the School Nurse informed of your child’s health status throughout the year. Should any changes arise please contact the health office.

NPI Kid’s Closet is open to any student who may be in need of clothes, shoes or coats. Please contact the health office for assistance.
Jefferson Elementary
Moscow Elementary
We would like your child to have a safe and healthy school year.

This form and medication needs to be brought to the School Nurse by an adult. Please do not send medication in with your child as this is a safety concern.
The only exception to the above policy is cough drops. Students may bring cough drops accompanied by a parent note that is effective for one week. Please send an allotment you would like your child to have for the day. Do not send the whole bag in at once.
Pack a change of clothes (undergarment, socks, a top and bottom) in your child’s bookbag. Accidents happen (spills, rips from playground, etc.) and we want your child to be as comfortable as possible throughout their day if they should need to get changed.
Please send your child with a water bottle as there are numerous water filling stations throughout the school.
Communication with your child’s classroom teacher is very important. We ask to please include the school nurse if it pertains to a health and/or illness issue so we can best support your child.
The PA Public School Code requires dental exams for 1st, 3rd, and 7th graders. You may submit a private exam or have your child examined at one of the school’s dental clinic days, which will be provided to you when scheduled. The private dental form is located on the school website under Health Service Forms.
