Initial Pre-participation Physical: Complete sections 1 to 7 for the first sports season of the school year
Recertification: For subsequent sports seasons complete sections 8 with a parent/guardian and student signature. Section 9 must be signed by a Physician if an injury or illness occurred in the previous sports season.
Wrestling: Section 10 must be completed
